Collection Publishing Request or Information Update Form

(example: uic_cfa or wiu_sequel)

Before requesting that a collection be published for the first time, please check that the following prerequisites have been met:

  1. Collections should be in accordance with the principles defined in the Consortium of Academic and Research Libraries in Illinois (CARLI) Digital Collections Collection Development Policy.
  2. There are five required metadata fields that must be included in all collections: Title, Collection, Language, Rights, and Type. 
    Specifications for these fields, and the information that should be contained in each of them, can be found at Required Metadata Fields for CARLI Digital Collections
  3. Each collection must have an "About This Collection" page that describes it.  Information may include, but is not limited to:
    1. The relationship between the collection and the institution
    2. Source of the original items
    3. Purpose of the collection
  4. The look and feel of a collection should be customized to reflect the institution or the specific collection. This customization should be completed prior to publication by using the customization form

Collection information:

Remember to click the the "Upload" button to attach the image to the form! Attach a JPG image to use as the representative image for the collection in the "by topic," "by media," "by name," and "by institution" results lists. Image dimensions must be exactly 300 x 300 pixels. If it is not, CARLI staff will crop it. To attach image, click the "Browse" button to select your image. Once selected, click the "Upload" button to upload image to this form.
Files must be less than 2 MB.
Allowed file types: gif jpg jpeg png.
Alternatively, You may choose an image WITHIN a collection and give us the URL to that image. CARLI staff will crop that image to 300 x 300 pixels. If an image is not provided, CARLI staff will choose one to represent the collection.
Please describe this collection in a summary statement of no more than 300 characters. This summary will be used in the "by topic," "by media," "by name," and "by institution" results lists.
If this collection does not fit into the Topic Categories that are already in the list above, please select "Other..." to enter new suggested categories. These suggestions will be reviewed by the Created Content Committee for addition.

List up to five metadata fields from your collection to use as "narrow your search by" options for the collection. Facets allow users to select options and further refine their search or browse results. (Each metadata field used as a facet must be set in Web Administration Field Properties to "Search=Yes" and "Hidden=No.")

Select which size options users will have to download images from your collection. Download options that are turned on by default are small (250 x 250 pixels) and medium (500 x 500 pixels). You may choose to remove the download option completely, or allow users to download your images in various sizes. Select ALL size options you wish users to see for your collection.

Please enter the text that will appear on the collection's home page if you have not already submitted it on your Collection Customization form. You may provide the text/information with HTML or other formatting coded in. If you would like to display an image, please provide the URL to an image that is hosted on your own server or a URL to an image in your collection. You may also upload an image to be used on your collection home page below.
If you wish, you may attach a JPG, GIF or PNG image to use in your collection home page. To attach image, click the "Browse" button to select your image. Once selected, click the "Upload" button to upload image to this form.
