CONTENTdm New User Names Form

CONTENTdm user names must be created by your own library personnel for use in the CARLI CONTENTdm hosted environment.

Note: This form must be submitted by an institution's Primary CONTENTdm Contact.

1. Each person at your institution that will be working in CONTENTdm must set up an individual, personalized OCLC WorldCat user name and password on the OCLC website. If you would like to create a general-use user name (e.g. "librarycdm" or "digi_staff") for your institution, you may do so but it will still need to be connected with an email address.

• Note that CONTENTdm user names must not contain the ampersand (&) or percent (%) characters.
• If a user already has an existing WorldCat.org user name, they may use it with hosted CONTENTdm if they wish.
• If you forgot your password, go to OCLC's password reset page.

2. The Primary CONTENTdm Contact must use this form to send CARLI only USER NAMES (do not send passwords). Indicate on this form which collections the user names should be allowed to access (or if all collections).

If you have more than 10 users to add, please submit this form more than once to accommodate all of your local users.
